On Line Event Registration

I am trying to setup an on line registration for a race in October.  I have the form created, but when I try to put in the information, it tells me I have already registered, and ask if I want to register another name. 

I tried to setup a fuzzy rule for this event, but that does not seem to work, and it continually prompts me to change the information.  Also, in the profile I turned off the required field for email address, but on the form it continues to display the email address field with an *.  Any help would be welcome
