Hey guys,
I see why it would behave as it does (but it should not).
Using CiviCRM 4.1.1. and drupal 7.15 and civimail.
If one of my users forwards any of the mailings (for whatever reason) to a friend who happens to be a member too, clicking on unsubscribe from the forward will unsubscribe the forwarder.
When the forwadee clicks on unsubcribe (optout), he is sent to the site with the message to unsubscribe listing the email of the forwarder. The text though is pretty small (the email listed) and can be missed pretty easily (and even if not, the forwadee can continue to click unsubscribe).
This only seems to happen if the forwadee is logged into the site. When not logged in, the parameters seem to get lost in the login process to unsubscribe for the forwardee.

Is there any way to force civimail to check the originating email (the parameters) and compare them to the logged in user before unsubscribing? I believe this was the original intention but fails in this setup.

This is a problem with every mail blast program and has more to do with the limitations of Email than of Civi. Not that we couldn't do better... some mail blast programs display the email to be unsubscribed in BIG BOLD TEXT to remind potential forwardees that they have followed the wrong link. Others take it one step further and require the user to actually type their email address in a form and compare that with the address on record before proceeding. Civi probably ought to look at one of those options.

I'm afraid I don't get your logged in/logged out scenario. Civi does not require a user to be logged in to unsubscribe from mailings. If you have somehow overridden this behavior and are forcing people to log in before unsubscribing (really, does everyone on your mailing list have an account? Really?) you should look at your implementation to see why params are getting lost.

This is actually a problem that I've been trying to resolve. The fact that optouts for me require login. I've for a while now been actually thinking this was intentional for civimail.
It actually is a bother for me as some of my users have complained when trying to opt out (Yes, most of my users actually do have accounts).
So, I need to take a look at my 'implementation' for sure but where though?
I did not specifically make a change that would have caused that behavior. I'm making the assumption that somehow civicrm wants me to allow access for anonymous users to civicrm to allow them access to the link from optout?

As for the forwards, I've included the {action.forward} token with a pretty 'seeable' message to use the link to forward instead of their mail forward button. And I can see its being used.

I don't feel entirely qualified to answer, but I can tell you that opt-out pages are supposed to be open to anonymous users.
Try pinging us on irc, someone may know of a permission or other setting/bug.

Correct. Always looking for the 'hard' answer when the permission 'CiviMail: access CiviMail subscribe/unsubscribe pages' is staring at me.
