Only Two Datepickers Working on Batch Update for Contributions

I have recently upgraded a site to CiviCRM 4.1.5 (driven by Drupal 7.15) and am having an issue where my datepickers are working across the site until I get to the "Batch Update for Contributions - Contribution update" page.

Here the first two datepickers, "Receipt Date" and "Receive Date" are working just fine, but the next one "Thank You Date" does not pop up and entering a date manually does not seem to work.

Any suggestions appreciated.

Can you look at the page using FireBug for Firefox and see if there are any errors on the Console or Net tabs?

When loading the Batch Update page with the datepickers on it, Firefox gives me this error:

Timestamp: 12-11-05 02:32:54 PM
Error: uncaught exception: Unexpected literal at position 2

Also the web page reports the following error:

Notice: Undefined index: payment_instrument_id in CRM_Core_BAO_UFGroup::setComponentDefaults() (line 2919 of /path/to/drupal/site/modules/civicrm/CRM/Core/BAO/UFGroup.php).
The datepickers are not the only things broken, unfortunately. The page is also incapable of doing Batch Update actions (I can use the first two datepickers as expected, but they are incapable of propagating to the other contacts when I click "Click to copy Receipt Date from row one to all rows."

This is, of course, the entire purpose of a Batch/Bulk Update page - to propagate values...

That first error looks like it. Can you click on it to see the actual line of code?

Unfortunately not. Firefox does not seem to be providing me with any additional information beyond what I posted.
