We recently updated this client to 3.4.8 and are running into an issue with Activities.

It seems to be issue related to saving the data into the DB.  When you are on the contact record and try adding an activity (we do have one custom activity, but problem with the standard ones as well). 

It says:   Activity has been saved. .

But then it does not show the new activity. 

In digging into it, we found that civicrm should save the data into a multiple tables, such as civicrm_activity and civicrm_activity_target (maybe there are more tables involved but only found these two).  For some reason the data is NOT saved in the civicrm_activity_target table.

However, if you go to the top menu and try to add an Activity from Contacts, New Activity, it works fine.  It shows up on the contact record. 

All the automatic activities for recording signing up for an event work fine.

Is this a bug? I could not find any solution during my searches or that it's a known issue.
