Basic cross tab report

I recently added a report to the extended reports extension which creates a single crosstab table of two custom fields - you can choose which but obviously not all are equally valid - you want the column headers field to be one with a small number of option values. The use case was Regions (custom field) in the columns & 'Ladder in the columns & looks like


Ladder Rung 1 Ladder Rung 2 Ladder Rung 3
Region 1 600 1000 60


 Region 2
 6000
 700
 9000


etc. A few filters are available.

The format is easy to extend onto other scenarios - but the requirement was as above.

How can we order the columns by, say, Ladder value - 1,2,3,4,5 across?
