Prepopulating custom fields on an activity

I'm using Civi to record Referral calls made to our company via Activities, and then schedule Follow up calls 30 days later. Households are entered with a parent and up to 6 children, and the activity "Follow Up" is made for the following month. There is a list of specific survey questions employees ask during the follow up call, which I have made custom fields for and attached them to the activity.

My question is, can Civi take the contact info from the Household's children and pre-fill fields in the activity's custom data? I have attached a screenshot of what the fields look like. Specifically I'm looking to pre-populate the fields "Child Name", "Child DOB", and "Child Age". I'm guessing this might not be possible without some backend wizardry, but thought I'd ask. Thanks! Please let me know if you need any clarification...

you can definitely do the below via hooks and setting the default values (u'll need php and civi api / mysql skills for this)

coleman and/or peted might have better ideas on how u can accomplish the below via webform_civicrm

you might also be able to get closer with  Webform-CiviCRM though am not clear if 'child' is a COntact or just a field
