Profile can't be accessed

I can't seem to get this profile to work.  Its visibility is Public Pages and Listings. When I include it in my contribution form, I get the error below.

Sorry but we are not able to provide this at the moment.
The requested Profile (gid=14) is disabled OR it is not configured to be used for 'Profile' listings in its Settings OR there is no Profile with that ID OR you do not have permission to access this profile. Please contact the site administrator if you need assistance.

What am I missing?  Is there an ACL I need to check?

Hi. Please update your Forum profile to show what CMS and what CiviCRM version you have.

Is your profile indeed configured to be used for 'Profile' listings?
