Big Troubles with Membership Renewals

Here is my issue:

Any/all members who are attempting to renew their memberships get an error, BUT it charges their credit card successfully.  But yet does not update their Memberships as appropriate, nor does it send email notifications to contact or admin.   

Any/all 'new' members (those that do not appear as contacts) work perfectly fine: Charges card, updates member record and transmits correspondence. 

Been struggling for awhile with this issue.   Upgrade did not resolve.   Changed Membership statuses, did not resolve.

Is there any way I can troubleshoot this?

Civi Version:  4.1.6
Membership Type:  Fixed Period, Fixed Start
