Further progress and challenges in civicase

I created two civicases with few sub-activities organised along a timeline.   The timeline reference date (open case date that I renamed completion date) is in the future. I have three activities before that date and two after. 
Everything seems to work fine.  Yet
1) when I click on audit I get the attached message.   I like to understand what I need to look at and whether, not being a php specialist, I can get through
2) when I add custom fields in the activities, then report and QA do not work and I get the message error on reporting attached in appendix.

The first issue looks like a bug. Can you upgrade to 4.2.8 and try again?

For the second one, can you enable debugging in CiviCRM and try again and post the full output if it still errs?
