Please forgive the noobish question, I am pulling my hair out.

We are asking for users to buy a membership and collecting signup information to create the contact. Although the signup form does ask for email address, there is no place to ask for the user preferred password. I am hoping someone will be kind enough to help me turn this back on. I had it working, don't remember what I did, and it has dissapeared sometime around the upgrade to beta 4.

I really appreciate your taking the time to shove me in the right direction.

I have been wrestling with the same issue for a couple days. I have one install where the account creation fields show up fine and one that they never do. After checking a lot of settings I found the issue in the WordPress general settings. Make sure you have the box 'Anyone can register' checked. This worked for me. Hope it helps.
