Case Intake Form

So I am very new to all this,but i am getting a better handle of it day by day.

I was asked today if there is any way to create an Activity "Intake" that could open an intake form [preferably one that can store in the information in the database for stats later]

I have read about the buildform hook but don't quite understand how to do.

Any help would be appreciated

Hi Drupal module webform_civicrm allows you to create a form that can create and update Cases - may be what you are after.

Webform gives you some nice extras but you can also just create custom fields for the open case activity (which is basically the "intake" activity) which then appear when you go to New Case.

If there are contact-record fields you want to collect then the "Name and Address" profile is also customizable so that they appear on the "create new individual" popup which you can choose on the New Case form, and get stored on the contact record. It depends if you want to track their values over multiple cases (like maybe job title) or whether they don't change (like birthdate).
