I have turned off civitheme already all other related CiviCRM modules.

Civi's still trying to run the query on civicrm_extensions that doesn't yet exist, and error-ing out before the upgrade begins.

I worked around by commenting out the public static function &getExtensions() in CRM/Core/PseudoConstant.php, then un-commenting it after the upgrade completed successfully.

Thanks for posting this fix @CiviTeacher.com!

If something can go wrong with a CiviCRM upgrade, it will... Not sure why this happened on this particular upgrade, but it did.
