Addressee for households

I have addressee for households set up as {contact.custom_25} {contact.custom_26} {contact.household_name} where the two custom fields are typically 'Mr & Mrs' and 'John & Jane' respectively.  This appears to be working for imported contacts - according to the summary screen - but emails to a household (using the sample CiviCRM newsletter) end up with just 'Dear Smith'.  Also when adding a contact in the normal way, for example a household called 'Jones' plus the 2 custom fields, then addressee stays as just the household name - grrr.  It appears that 'contact.display_name' isn't getting updated.  BTW, postal greeting seems fine as printing labels gets the right result. I have run the update greeting job.

I guess I'm not understanding how households work as far as addressee goes.  Can anyone throw some light on this, please?
