Premiums by Pledge Amount

We are running a fundraiser that says if someone pledges to donate at least $5000 over a 3 year period, they can choose to have their donation be part of a special program. I am using a premium but would like the premium to go by the total amount someone is pledging versus the amount they are contributing "today." - See the form here: http://manchestervtlibrary.org/donate/

Is this possible?
