URGENT: Cannot find a way to delete a survey/petition

CiviCRM 4.1.5

When I try to schedule an activity (Phone Call), I am getting a section on the form for survey questions. (Maybe it's a petition). Some of the fields are required so I get an error when I try to save the phone call activity. I cannot find a way to delete these questions and I can't even find the survey/petition on the dashboard. I tried disabling the CiviCampaign component, but the questions still appear.

How do I get rid of these question?

Hmm, probably you shouldn't be using non-survey activities (especially one as common as "Phone Call") in your surveys.
To fix your problem:
Go to Campaigns->Surveys and edit whichever one is using that activity type.
On the profiles tab, remove whatever profiles you have added.
Try asking your question on the new CiviCRM help site.

There are no surveys listed in Campaigns->Surveys

The problem, I found later, was that I created a set of custom fields for the survey I intended to use and these appeared when I tried to use the phone call activity. I did not complete setting this survey up because there was a bug that needed to be patched (backported) so I ended up using Survey Monkey instead.
