troubleshooting error when submitting new member form

I am troubleshooting a problem with a new membership form

Upon submission, validation does not fire - well, in some browsers - and you are just taken to the home page.  When this happens, no error is logged in /public_html/media/civicrm/ConfigAndLog or anywhere else that I can find either.

How to troubleshoot this?

Enable JS Debugging in whatever browser it is and look for JS errors. Tell us which browser and we can send more specific instructions.
