Civievent: DB error when adding/editing participants

I have set up a civievent on our wordpress site.  Our first participant successfully registered.  However, I cannot, as an admin, go in and register participants or add notes to a currently registered participant without getting the following error:

Sorry but we are not able to provide this at the moment.
DB Error: constraint violation
Return to home page.

Ideas?
