Civimail opt-out options

My marketing person just asked me if there's a way to have patrons select WHY they are opting out, which I reckon is basically adding a form page for the opt-out to go to? Is this possible? (I know, anything is possible with a good dev...but is it possible out of the box, so to speak?)

no support for this out of the box. It is probably a relatively small and easy extension to get a developer to do (3-8 hours?)
