Two Smarty Errors after Upgrading to 4.3.8

After a "successful" upgrade from 4.1.5 to 4.3.8 on a D6 site, the contact name, inline editing and the Actions button on individual contact records is missing or not visible. I'm seeing two errors in the log files, which I think is related to the issues above:

1. Smarty error: unable to read resource: CRM/Contact/Form/ActionsButton.tpl /civicrm/packages/Smarty/Smarty.class.php on line 1094
2. Smarty error: unable to read resource: CRM/Contact/Page/View/Demographics.tpl /civicrm/packages/Smarty/Smarty.class.php on line 1094

I found a related post here: http://forum.civicrm.org/index.php?topic=26168.0.

This site is running a custom module (drupal side) and the Summary Fields extension. I disabled the custom module and removed both the templates_c and ConfigAndLog folders to no avail.

Any other suggestions?

Do you have any customized templates? Did you clear Drupal caches?

No custom templates within Civi itself. There is a custom module enabled on the Drupal side that calls to custom templates located in their own folder under /sites/default. But I disabled that module and then cleared all caches and the issues persist.

The Summary Fields Civi extension is also being used. Disabled that. Cleared caches. Still no luck.
