Using CiviCRM with WordPress and Capability Manager Enhanced. I created a new WP role ("Therapist") with Capacity Manage and left the level at the default '0' (= "Subscriber", I assume). I then created a bunch of permissions at Administer>Users & Permissions. But even with these I can't log in as one of these "Therapist" users. I get "You do not have sufficient permissions to access this page."

Permissions granted:
CiviCRM: access CiviCRM                           
CiviCRM: add contacts                                 
CiviCRM: view all contacts                                                
CiviCRM: edit all contacts                                                   
CiviCRM: access all custom data (previously set in role creation)
CiviCRM: access CiviCRM                                                
CiviCase:  access my cases and activities                                 
CiviCase:  add cases             

Do I have to select a higher "level"? Or is there some other set of WP permissions that I can assign to "Therapists" to make this work without giving them more administrative permissions than I want to?

Follow-up:
Leaving the 'level' at '0' and adding 'Read' permission under "Other WordPress core capabilities" seems at least to get me to the admin panel and from there into CiviCRM.

You are correct that the WordPress level setting is deprecated.  Capability Manager Enhanced in no way imposed or "still uses" them in an antiquated sense that interferes with the current permissions model.  In general, I provide access to the setting and leave 3rd party code to decide if or how to use the value.  Although the "simple hierarchy" roles model is rightfully superseded, I find that it is still sufficient and expedient for some use cases.  For example, default role definitions are such that only Administrators can edit capabilities.  However, if the manage_capabilities cap has been added to other roles, Capability Manager Enhanced itself will prevent the logged user from editing roles of a higher level than their own.
