db upgrade scripts failed

I am upgrading a site from 4.1.1 to 4.4.5.  When I launch the upgrade via the web page page, it fails with:

[Error: Finish Upgrade DB to 4.1.2]
Error 64: require_once(): Failed opening required 'CRM/Grant/PseudoConstant.php'  ...

Two odd things here:
a.  I did the same upgrade on a sandbox clone of the production site and it went flawlessly.  The sandbox is on the same host and was cloned from the same data and settings.

b.  When I check some of my other sites running 4.4.x, none of them have the file CRM/Grant/PseudoConstant.php, although the file is present in other paths (e.g. Core, Case, ...).

Suggestions?   What do I need to do to get the upgrade to succeed.

I'm not sure what happened, but perhaps try to upgrade to 4.1.2 and then to 4.4.5.
