[FIXED] Tried to upgrade to 4.5 Beta - failed (again)

My upgrade attempt from 4.4.5 to 4.5 Alpha 2 lasted about 2 seconds - not very helpful diagnostic screen attached.  I restored the program files and I am still at 4.4.5.   Anyone have any ideas?

Sure, here it is, attached.  It looks like some kind of pre-existing cron problem to me, so I'm not sure it will help.

Clarkac : Can you please check again by - session reset(logout/login ) > cache clear(Ctrl+Shift+Del) > Hit the upgrade script.

Even I had also encountered this issue earlier on upgrade, but then it was not 4.5alpha2 upgrade specific. I did what I mentioned above and did works for me.  :)

Tried that - failed exactly as before.  I then switched on debugging (back trace was already on) and tried again, after clearing caches.  Failed again.  Cleared browser cache & Civi cache - failed again.  Nothing new in the ConfigAndLog folder.  BTW, the error screen options of 'Retry' and 'Skip' do nothing at all, and I am still at 4.4.5 level, so the upgrade didn't appear to do anything at all.

Before upgrade can you please try clearing you templates_c folder and check if that helps.

I deleted all the files in templates_c folder but the result was exactly the same.  By the way, in case you were wondering what kind of weird setup I have, this is a test system that was recently used for a user test, without any problems.  Following this upgrade error message, when I restore the program files (not the database) I am still at 4.4.5.  So it's choking somewhere right at the beginning.

As beta 1 was released I tried this.  Same result, but this time it appeared that the database was updated to 4.5.  But the menu isn't working properly & I suspect something is screwed, so I will restore the database and start over.  I'll let you know progress.

I restored my database which was at 4.3.8, and upgraded to 4.4.6 and tested that it was working Ok - it was.  I then installed 4.5 beta 1 and ran the upgrade.  Same problem - after a very few seconds I get 2 buttons - Retry and Skip neither of which do anything.  Something in the upgrade process is broken... Is there anything I can do to help diagnose what it is?

After 4.5 beta 2 was released I tried upgrading again and voila! upgrade worked.  I am now on 4.5 beta 2.  Thanks!  Now I can test...

As this problem had previously disappeared when I tried beta 2, I thought I'd go back to 4.4.6 and try again with beta 6 to make sure it was really fixed.  So I restored to 4.4.6, made sure 4.4.6 was working OK then started the upgrade, hoping to see the upgrade progression though beta 1 to 6.  But it crashed exactly as originally after a couple of seconds - screen attached.  Did something get regressed from beta 2 to beta 6?
PS. There's nothing in ConfigAndLog

I tried to upgrade again with Beta 7, after I am sure that my 4.4.6 system was working well.  Failed again, exactly the same way (see screen attached to previous msg).  I carefully followed the upgrade instructions.

The end result is the screen you see, but the resulting system seems to be a partly upgraded 4.5, so I tried a simple search by using the url and got a crash (backtrace attached).  Perhaps this will help? It looks like the problem is a missing field formal_title in the contact table?

There's been no reaction to this problem - which is worrying because unless this is fixed maybe other upgrades will fail the same way? (to state the obvious).

I have a backup of my 4.4.6 system, so this is repeatable.
