Drupal 6 Membership Role Sync gives DB error [Solved]

Resolution was to reinstall civicrm, this was fixed...but not sure if I would call this resolved as i am not sure what caused the problem in the first place. Just another trying "to turn it off and on again"

Hey all so I working with a drupal 6 dev environment that has the membership roles of the drupal users. I am trying to sync them to a membership type that is setup through an Association Rule with the status setup as well. When I select the CiviCRM membership type and then the Drupal Role with the check off the Current and  Canceled I then click submit and received  the following message

There was an error adding the association rule. Please check your database settings and try again. If you continue to get this error message then try to reinstall CiviMember Roles Sync.
The message is not clear on resolution. I checed the DB settings in civicrm.settings.php and for drupal in settings.php and both check out fine. The only other error I can see is

Which I am not sure it relates cause it does not show up in the logs when I tried to activate the Association Rule nor does it show up in the ConfigAndLog of civicrm. I am perplexed...please help
