Here's the website request:

I would like to use a contribution form with profile for kids to fill in personal requests where kids can request help (homework, companion if depressed, etc). Basically it is an online kids help line. Then when the request is submitted, a csv file is created with the info that will then be sent via cron job to another system for the volunteers to get the info. They'll get it on their phones and be able to mobilize pronto.

My plan is to do a contribution form with profile. Is there a way a csv file of the single request can be made upon submit?
