Urgent - Duplicate Contribution records created

Hi
I have a major issue in that CIVICRM is creating duplicate records for a single contribution entry.

From the user contribution page

Amount   Type   Source                            Received                       Thank-you Sent   Status
$20.00    Donation - Total Amount      October 3rd, 2014 10:30 AM               Completed
$20.00    Donation - Total Amount      October 3rd, 2014 10:30 AM                Cancelled

The duplicate record was caught and cancelled but this is worrisome - how many did we miss?

Help please!

The confirmation page on (see the bottom of Title and Settings Tab) can help prevent duplication.  Otherwise when someone clicks they button twice it can charge them twice in versions 4.4 and prior if contribution "confirmation page" is turned OFF. 

If you cannot stand the confirmation page, upgrade to 4.5.0.

The suggested solution seems to be specific to online user entered contribution, however, the issue is with admin entered contributions entered for cash received on the contribution entry screen. Is there a confirmation page for that?
