Duplicate Main Email entries

We have a website where duplicate email entries are being entered/generated in some way.  The example contact does have what seems like a high number of updates.  The client doesn't have any idea what could have caused this.  I have been tasked with figuring this out but I don't have a clue.  If anyone else has seen this or has a clue to give it would be very much appreciated :)

Probably not important but 3 of the 5 Main emails have signatures.  Also I don't know how many contacts are affected.

Is there anything on the Change Log tabs or Activities tab that could account for this?

If this is still happening, and turning on binary logging is an option in your server environment, that's a very quick way to get to the bottom of things.

Nothing in the Activities tab.  There are what seem to be an excessive number of entries in the change log, most by the contact.  I looked for more info about the changes but I wasn't able to find anything.  I looked in the database.  I will look into the binary log option.  Thanks.
