Drupal 7.34, CiviCRM 4.5.4 (recently updated from 4.3.

I am trying to understand this error.  We have a client that seems to be getting these errors more often.  The update to 4.5.4 happened on 2/5 and a server update was performed 2/11.  We aren't responsible for their server but can get information about what exactly was updated.

This morning 3 mailings were sent and there were 12 of these errors for each one.  It doesn't look like they were the same emails.  The actual emails sent were pretty much the same format, etc.

I looked at the code where this error is reported and see there is a path to this condition but I don't understand how it could get on that path.  I usually have to use a debugger to understand Civi code.

Any insights, suggestions, etc. are appreciated.

Added:  Could the IDS check have anything to do with this?  I am noticing a lot of errors from that.  I have turned off the check for authenticated users.  We have had trouble with that permission before.
