i have 4 group in my site.
1. activity
2. Patient Care
3. Administrator
4. Food and Diet

each contact can only follow one group,
so i want to create a custom field where the list in that custom field take from those group(s).

i see default a field in civicrm contact, but it allow contact follow more one group

any one experienced for this case ?

You could create a custom field with those 4 values and then create 4 smart groups based on that field. Those with access to those admin screens need to know to change your custom field rather than add groups directly.

i may be misunderstanding but if you want a form where visitors or admins can be forced to only add a contact to one out of 4 groups, you could do it on a webform and set the widget to be a radio button i believe
