4.6 Beta 4 Display preferences - key items not checked on install

I just did a fresh install of beta 4 (about 10 minutes before beta 5 arrived!) and noticed that the first column of items in 'Display Preferences' were not checked.  This is the column starting 'Prefix' and ending with 'Suffix'.  I'm sure this must be an omission or glitch and should be corrected.  I guess it's the kind of thing you wouldn't notice on an upgrade.

Hmm, there does seem to be something up with that form... I'm seeing php notices on it...
May be related to https://issues.civicrm.org/jira/browse/CRM-14974
