Credit card fields not showing on registration page

We're having an issue with the event registration that seems to have started in the last couple of weeks. When someone goes to the registration page they no longer see any of the fields where they would enter their credit card information. They fill out the form and hit 'Continue' and get the following message:

Please correct the following errors in the form fields below:
Card Number is a required field.
Security Code is a required field.
Expiration Date is a required field
. 
The input fields for their credit card now show, the very fields that were not on the form the first time they visited it. I'm not sure what change might have been made to cause this, I just noticed the issue last week.

If you can't link the problem to an upgrade in CiviCRM then have updated your CMS or installed new modules/plugins/extensions or has you host updated the server environment?

Enabling Firebug in Firefox or DevTools in Chrome could provide some guidance as to what is going wrong.
