update to hook specification docs

I've significantly updated the hook specification page with more structured layout and addition of 2.1 hooks

There are some hooks that haven't yet been commented in the source code so there are a number of blanks to fill in. Someone familiar to the new hooks could add a description sentence for each on the wiki entry if they feel so inclined. :)
