Hi Folks! I'm new to CiviEvent and am wondering if there is a way to include custom fields during the event registration process (before checkout), such as:

Checkboxes for questions like:

Do you have:

  Lower Back Pain [X]     Osteoarthritis [X]      Arthritis [X]    Diabetes  [X]

---

or questionaires like:

How long have you been dancing?
