import process imported phantom/blank records

I was doing a test import of 4 records.  In step 3 it said there were 4 records ready to import.  Step 4 (completion) said 12 records had been imported.  I go to contacts and I see the four new records and four blank ones.

Any ideas?

Were you importing Individuals or what - were any of the other columns possibly interpreted as involving Orgn or Household data. I have had similar odd reporting of numbers, but never before had blank records created?
