how to get custom data fields to appear in default contribution data entry form

so, i got some insight into this from IRC which was got me started, but i'm still a bit confused: 

So far I have three custom data fields created that are linked to contributions.  I'd like to know how to get one (or possibly all three) of them to appear in the default "new contribution" data entry form. 

Here's what I think i should do (in civi 2.1) : 
1) create any/all custom data group/fields
2) create a profile that includes them
3) go to administer > civiContribute > manage Contribution pages   
4) choose the appropriate page, then configure to include the profile i just created

It's step 4) that I'm stuck on - the pages listed here don't appear to include the default 'record a new contribution' form - they seem to be pages that came with sample data..

So, what am I missing?  or , is there not a front-end way to do this?  if that's the case is this  -  http://wiki.civicrm.org/confluence/display/CRMDOC/Customize+Built-in+and+Profile+Screens  - the solution?

You have to include the profile with configured fields in Manage Contribution Pages → Include Profiles,

You can check http://sandbox.civicrm.org/civicrm/contribute/transact?reset=1&action=preview&id=1
where a "contribution profile" has been configured with custom data.

but i'm afraid I still don't follow.  When I go to the "Manage contribution pages" area I don't see an option to "include profile."  rather, there's a list of existing contribution pages - these seem to be sample pages - and an option to create a new contribution page.  But what I want to do is add some profile elements (my custom data fields) to the *default* "record a new contribution" form, so that when staff are entering information about a gift, our custom fields are available in the main contribution area.  So.. I still don't understand how to link the profile/custom data fields i created to that default data entry form.  I must be missing something obvious?  or does this require custom code like the link i posted earlier suggests?

Hmm, i understand how that would help if I wanted to add a profile to a custom contribution page, but I want my custom fields to appear in the main form for entering a contribution - in the sandbox it's here - http://sandbox.civicrm.org/civicrm/contact/view/contribution?reset=1&action=add&cid=80&context=contribution 

So in the sandbox as of now, someone managed to get some custom fields - in  a group called  "Contribution data" with the fields "contribution text" and "contrib date" to print in this form.  But I don't understand how to do that using the process you're suggesting, because the only "contribution pages" available to edit are

None of which seem to reference this default form for entering a one time contribution for a contact...
should there be another contribution page visible?  maybe it's a global configuration setting that i'm missing?

>> Administer >> Custom Data - in the sandbox you will see they have created a Custom Group called Contribution Data which is 'used for' contributions - hence it appears on the Contribution Tab. HTH

thanks peter et al for the help.. apparently the situation is that when the custom data fields were first created rather than designating them as "used for" = contributions they were designated "used for" = contribution donations .. I just didn't realize you had to choose "donation" as the contribution type for the fields to appear in the default form.  Doh :)  Now the task is to figure out whether there will ever be a need to link event fees and other down-the-road contribution types to our custom data fields..  hopefully not so we don't have to do more multiple choice option entry.
