Drupal custom profile needs CiviCRM output

Hi, I'm attempting to get the below URL documentation to work on 5.x version of Drupal

I really just need the name and address variable pulled up...

Any help would be grandly appreciated.

I think the only change u need to make is:

module_exist => module_exists

and it should work in drupal 5.x
