I don't know if CiviCRM can be setup to do this or if I should find a Drupal module to accomplish this:
I would like to setup a simple magazine subscription for the organization's journal. Only non-members will use this since members already get a free subscription. On top of the 1-year subscription people can purchase single back issues.

I attempted to set this up with the Contribution Page set up with some fixed contribution amounts but they just show up as radio buttons and not check boxes (or at least the option for multiple selections).

I also need to grab two different addresses - the billing address and the mailing address if different.

I wanted this setup through CiviCRM since almost every other part of the site is setup with CiviCRM.

Is there a good example of a solution posted in the forum (i looked but didn't find)?

You can include a "Profile" with address fields in your online contribution page configuration (for the Mailing address). The one drawback is that you'd have to customize the template to add a Jscript convenience feature (e.g. "click here if mailing address is same as billing address" - which copies the values from one set of fields to the other).

HOWEVER, I don't think there's any way without some non-trivial coding to support the "checkboxes for back-issues" feature with auto calc'd amounts. Possibly using some combination of the new (2.2) buildAmount hook and post hooks ??? Event registration has a Price Set feature which would do what you need pretty nicely - but that's for events so probably would need to customize the messages / templates quite a bit to make it look like a fit.

Your best bet might be using UberCart with the CiviCRM connector module - although I'm not sure if that's been updated for the current CiviCRM / Drupal releases yet - search this forum / the web...
