I am running 2.1.4
I have a group whose members can login to civicrm without problem.
Now I add a new contact to the group. This new contact is set for allowed to login
When I try to login with this user, I am rejected (not allowed to login, call administrator).

I find that two contacts with exactly the same myopenid, the same name but written once with capital once with all small letters exist. I find that in civicrm_uf_match one user is entered, the other not. By manually removing the duplicate entries in uf_match and civicrm_openid I manage to make it work.

I assume that this is not the standard procedure.  There is a simmilar forum entry "User login fails on standalone using openid + acl".

When I analyse the code civicrm_uf_match makes the link between user id and myopenid url. The problem seems to be that by adding the new user the entry in the uf_match gets not updated and on top that the system does not tell that there are two users with the same openid url. Whenever an openid is added for a user the system should not accept it when the same opid url exists allready. Additionally if an openid is removed from a user the entry in the db should be removed!

How to avoid a similar situation?

It does seem like you've identified a bug. Can you report it in the issue-tracking system? http://issues.civicrm.org/
