Getting two sign-up fields when only one is needed

I seem to be getting two sign-up fileds in areas such as join an event. Only one is needed. Any clues how to remove one of these two sign-up fields?

Might help to attach a screen-shot and reference the URL path where the problem is occuring (e.g. civicrm/event/register) - not sure what you mean by two sign-up fields.
