I have been having a problem where when I search the database for records, and use the actions menu to add them to an event, multiple records are created. It will create a number of participants from one of the records or another.

Additionally when I search for a records based on a certain criteria (like source code), and add them to a group from the events menu, and then create a new group for them to be added to, it adds more records than are selected. For example I have selected 297 records, I get the confirmation message 297 records have been added to your group, 3 records have not been added (already in group)

 I haven't been able to reproduce this on the demo, I believe it's a problem with our site specifically, but I can't identify what.  Also, I can't tell if the two issues are related.
