CiviCRM Profile not storing data

I'm using the default CiviCRM Profile that I have modified to use as an email list signup.  I've added a few fields (like Contact Apartment #) for apartments, however, for some reason some of the fields aren't being stored.  The default Contact (Street Address) field is always empty, even though its a required field to sign up.

I've created a menu link so that people who have registered for the email list can go in and modify their settings using the Menu Manager and CiviCRM Profiles > Edit.  When users click on that link, it takes them to the edit page, but the address field, city, and postal code are all empty.  Entering the information again and clicking save says they've been updated, but if they click the Edit Profile link again, the fields are empty again.

2. does the profile work as from the backend?

1.  The information does not get saved in the backend.
2.  Not sure I understand this question. If you're asking if I can add the information in the backend, I can't.  It does the same thing -- I click on edit, enter the information, click "Save" and the page says "Your Individual contact record has been saved." but none of the information is added to the contact's information.
