Odd problem with household entry

We have started having a problem with entry of new contacts.  The scenario is as follows:  a new individual is created, and you want to create a household at the same time.  So you click the Use Household Address box, and enter the household name.  The search fails, so you are creating a new household.  Click save, and it creates both the individual and the household.  The problem is that sometimes the household name ends up including the full street address.  The household name then looks exactly like the format you see in the ajax search box for finding the household.   This is hard to describe, but please see the attached screenshot.  The household name was entered as "Cole-Stephens".  I have not actually seen this happen so I cannot be sure that it is not a user error of some sort but I keep finding these erroneous records in our DB.  The volunteer data entry people say they have not done anything unusual.  Any suggestions, please let me know.

Hi, I tried to duplicate this on my site and didn't have any luck.
Did you try and replicate this yourself?
Have you tried it on the demo?
Have you looked at all the data with the particular household names like that to see if they have anything in common?
