Bad URLs in clickthroughs

I'm using the clickthrough tracker and getting a white screen for some of the links.

A look at the stored URLs in the database shows me that there is bad data of various forms. Here's what I've found:

href="http://www.company.org/
http://
http://person@company.org

How are these getting stored like this?
Are some of these down to user error? The email one looks like a bug in CiviCRM to me.

Where is the code that handles processing emails to replace URLs with clickthroughs?
