Is there a way to add an automatic reference number to each new case?

When a new case is started a field is present on the form which takes the next number from the DB. If the case is deleted without any entries this number would return to the pool. If the case is deleted with data this number would not return to the pool.

Any advice appreciated.

Nope, it is not possible. Also currently CiviCRM don't have any custom data of type Case. So I am not sure how you will able to do this.

As I understand Drupal, is it not possible to put the module in a "mirror/parallel" location so I can make/test changes without affecting the "core " module (which I can switch back to if I abandon any changes)?

Cases ARE automatically assigned a unique incrementing Case ID (which is displayed in case listings and on the Manage Case screen).

For example, on our pubic demo - after logging in, look at:
http://drupal.demo.civicrm.org/civicrm/case?reset=1

Regarding "parallel" modules, as far as I know you can not have two different "versions" of CiviCRM running under a single Drupal site. However, you can create / configure directories for customized versions of CiviCRM's PHP and template files - and put modified versions of the code in those locations. See "Directories" under Global Settings.
