CiviMail From Address after Upgrade

I upgraded to 2.2.5 today. All seems good except when I go to create a CiviMail. I have defined default addresses in Administer CiviCRM > CiviMail > From Email Addresses

But when I go to the third screen of the wizard I see the screenshot that I have attached...no drop down for email from address nor the override VERP address option.

I can't recreate on demo as the demo works properly with the dropdown. I checked Drupal permissions and don't see any permission specific to this. Am I missing something or is this not working properly? How can I fix?

Hi Emily - Wondering if this is a cached template problem. Did you run directoryCleanup debug command and / or delete your templates_c folder?

thanks dave. it was actually a custom template which caused me to submit an improvement request to help me avoid this in future: http://issues.civicrm.org/jira/browse/CRM-4644
