Can anyone offer advice on how to set Civi up to allow people to fill in a "Grant" forum, let Civi gather some information, have them fax or email additional information.

We provide travel assistance, as well as funding for brain tumor research, clinical trials, and provide travel assistance to qualifed patients.  I would like Civi to handle this, but can't figure out how to create a "form" for them to fill out depending on the grant they are applying for.
