Billing name and address fields

Is there a way to turn off the Billing Middle Name field and Country pulldown on the contribution pages?

All of our contributors are from the U.S.  We've used Global Settings localization so that United States is the only country that shows up, but people still have to do the pulldown and there doesn't seem any need for that in our case.

I guess you will have to modify template file for this. ( templates/CRM/Core/BillingBlock.tpl )
Also you might want to check if your payment processor processes transaction without passing country in billing address.
