Why is the standalone activity form updating an old activity instead of creating

Hi,

I have modified the activity template, and created a activity type php file.

/civicrm/activity?action=add&reset=1&context=standalone&atype=12

It works fine, BUT it doesn't create a new activity, but always update an old one.

Any suggestion or where to dig ? (just tried on the demo, works fine, "unfortunately").
