Kurund helped me to solve this problem.

http://domain.org/civicrm/admin/setting/preferences/address

{street_address}
{supplemental_address_1}
{supplemental_address_2}
{city}{, }{state_province}{ }{postal_code}
{country}


Are required to be in {}.  For some reason these values were not in {} and so only the labels were showing.
