Install on IIS with a auto filled form ?

I am looking around for something that will work within the following  ideals

* User sends form that includes a pre filled drop down country/region  selection possibly dynamic eg Uk inserts UK regions
* form also requires entry of manually written numeric field, 2 x text area and then the usual fields.

can the form then put the entries into the database without registering.
form needs to be able to be marked pending /

Admin is emailed or pulls off report/for entries

or is civic a bit overkill for that?
