"Page not found" trying to run upgrade script

From 2.1 to 2.2.

Followed the upgrade instructions step by step. Three times in the last 24 hours.

Drupal 6.13
SQL 5.0.81
PHP 5.24

I've tried the upgrade url spelled out in the instructions. I've tried adding /sites/all/modules to the url path. I've tried with and without the "www" in the URL.  I've tried the non-clean url with those above variations.
My worst fear is that the rhythm really is going to get me.

Where does the upgrade url lead to? Should there be a file in the civicrm or civicrm/install folder?

It used to be upgrade.php, but that's not there anymore.
My worst fear is that the rhythm really is going to get me.

civicrm/upgrade url is map to 'CRM/Upgrade/Page/Upgrade.php' file and it is located in civicrm code base.
